Data Controller is Just Jump s.r.l. The data provided will be processed by Just Jump s.r.l. also electronically, for the archiving of this form. The Data Protection Officer is the legal representative of the company. Data will not be communicated to third parties, nor will it be disseminated.
The provision of data is mandatory for the performance of the required administrative tasks. In case of refusal to provide the requested data, consent to access the facility cannot be given. Pursuant to the articles of Legislative Decree 51/2018, the applicant obtains, upon request, access to their data, extraction and information about them and may, if the legal requirements are met, request their updating, cancellation, transformation into anonymous form or blocking.

1. I declare that I am in good physical and mental condition to participate in recreational or non-competitive sports activities;
2. I declare that I am not under the influence of drugs, that I have not overindulged in food or drink, and that I have not taken and will not take any psychotropic or narcotic substances in the 48 hours prior to the activity;
3. I am aware of the foreseeable and unforeseeable risks associated with participating in this activity;
4. I have read and understood the rules of the Just Jump facility and the imposed regulations, and I fully accept all parts thereof. I also accept the right of the staff to remove me from the facility in case of non-compliance with the rules or instructions given by the staff;
5. I hereby assume any and all liability for myself, for personal injuries and/or damage caused to others (and/or to property) as a result of my conduct not in conformity with these rules, and for my heirs and/or assigns, to release Just Jump, its collaborators and/or employees, as well as their heirs and/or assigns from any liability for injuries, and//or any damage (even caused by third parties), which I may suffer in connection with and as a result of the activity carried out at the Just Jump facility;
6. I have carefully read and evaluated the contents of this document and have clearly understood the meaning of each individual point before signing it;
7. The Participant acknowledges that closed-circuit television cameras may be active in the JUST JUMP premises and therefore consents to the use of images and videos for security purposes;
8. The Participant acknowledges and understands that other individuals at the Just Jump facility may use or operate and/or use photographic and video recording devices, and notes that Just Jump has no control over them, and is not responsible for the images or videos that may be reproduced by such devices, and how such images and videos may be used, processed or disclosed by third parties. The user also consents to the possibility that Just Jump staff may use photographic and/or video recording devices, during the exercise of the activities carried out within the facility, for promotional and//or advertising purposes.
I fully understand and agree with the purpose of these rules established for my safety and that failure to comply with them may put both myself and my companions in danger. For the purposes of articles 1341 and 1342 of the Civil Code, I declare that I specifically approve points 1, 2, 3, 4, 5, 6, 7, 8 of this document.
